Springfield And The Ozarks
48 Hour Film Challenge
The undersigned,
Proprietor of the location or property used in the Film, of which the
undersigned is the owner and/or manager with the power to so do, grants the
appearance of said Property in the Film being produced by Team Producer as part
of the SATO 48 Springfield And The Ozarks 48 Hour Film Challenge – 2008.
VIDEO SHOOT RELEASE: The undersigned hereby irrevocably grants to the
Team Producer, and to the SATO 48 Hour Film Challenge (Hlabeard Productions,
Inc.), its licensees, agents, successors and assigns, the right, but not the
obligation, in perpetuity throughout the world and in all media, now or
hereafter known, to use (in any manner Team Producer and/or Hlabeard Productions,
Inc., deems appropriate, and without limitation) in and in connection with the Film,
by whatever means exhibited, advertised or exploited: the appearance of the
Property in the Film, still photographs of the Property, the undersigned’s appearance,
still photographs of the undersigned, recordings of the undersigned’s voice
taken or made, and the undersigned’s actual or fictitious name.
Furthermore, the
undersigned acknowledges that Hlabeard Productions, Inc., or its assigns may
shoot behind the scenes “making of the SATO 48 – Springfield And The Ozarks 48
Hour Film Challenge” footage, for use in a motion picture, and the undersigned hereby
irrevocably grants to the SATO 48 – Springfield And The Ozarks 48 Hour Film
Challenge (Hlabeard Productions, Inc.), its licensees, agents, successors and
assigns, the right, but not the obligation, in perpetuity throughout the world
and in all media, now or hereafter known, to use (in any manner Hlabeard
Productions, Inc., deems appropriate, and without limitation) in and in
connection with the motion picture, by whatever means exhibited, advertised or
exploited: the appearance of the Property in the motion picture, still
photographs of the Property, the undersigned’s appearance, still photographs of
the undersigned, recordings of the undersigned’s voice taken or made, and the
undersigned’s actual or fictitious name.
LIMITED LIABILITY RELEASE: Furthermore, the undersigned understands that
during the undersigned’s participation in the SATO 48 Hour Film Project, the
undersigned may be exposed to a variety of hazards and risks, foreseen and
unforeseen, that may include, but are not limited to, personal injury, property
damage and death. To the fullest extent allowed by law, the undersigned agrees
to waive, discharge claims and release from liability the SATO 48 – Springfield
And The Ozarks 48 Hour Film Challenge (Hlabeard Productions, Inc.), its
officers, directors, employees, agents and leaders from any and all liability
on account of, or in any way resulting from injuries or damages. The
undersigned understands and intends that this assumption of risk and release is
binding upon the undersigned’s heirs, executors, administrators and assigns,
and includes any minors accompanying the undersigned on the Challenge.
